In compliance with the provisions of Ministry of Health Administrative Order No. 71, s. 1983 and pursuant to the provisions of B.P. Blg. 325 effective October 10, 1983 charges and fees for quarantine services rendered shall be revised as follows:

1.       Immunization Fees -

a.            Cholera - P20.00

b.            Yellow Fever - P60.00

c.            Other immunizations - P15.00

d.            Issuance of duplicate of vaccination certificate - P15.00

2.       Fees for Fumigation Services -

a.            Issuance of deratting exemption certificates (based on net registered tonnage of vessels) - P120.00-550.00

b.            Issuance of an extension of a deratting exemption certificate - P120.00

c.            Issuance of a deratting certificate (charge for quantity of fumigants used)

(1)   Hydrocyanic - P5.80/oz

(2)   Sulphur - P7.80/oz

3.       Physical examination fee for complete medical examination - P60.00-125.00

This Order supersedes all other orders and issuances on the same subject matter.
