AMENDMENT OF ITEM II (B) OF PRC RESOLUTION NO. 2003-159 (A) DATED DECEMBER 1, 2003 RE: REQUIRING APPLICANTS IN LICENSURE EXAMINATIONS TO SUBMIT CERTIFICATE OF LIVE BIRTH PRINTED ON NATIONAL STATISTICS OFFICE (NSO) SECURITY PAPERS



WHEREAS, in Resolution No. 2003-159 (A) dated December 1, 2003, the Commission resolved to amend Item II (b) of PRC Resolution No. 2002-76, series of 2002, requiring the submission of the following:

b.1) Original and Photocopy of the Certificates of Live Birth printed on National Statistics Office (NSO) security paper and signed by Ms. Carmelita N. Ericta with attached original receipt.

b.2) Original and Photocopy of the computer generated NSO Certificates of Live Birth stating at the dorsal portion thereof, "A Certified True Copy: Office of the Civil Registrar General" duly signed by an authorized NSO statistician in security paper with attached original receipt.

WHEREAS, there are applicant in the licensure examinations wherein their names do not appear in the NSO database, and are only issued a negative certification on no record of birth by the NSO.  Hence, their application for licensure examination are denied by the Commission for failing to comply with the requirements, though they possess valid birth certificates issued by the local civil registrars.

WHEREAS, the strict implementation of the aforementioned requirement would work to the prejudice of, and a denial of the opportunity for, an aspiring professional to take the licensure examination.

WHEREFORE, the Commission RESOLVES, as it is hereby RESOLVED to amend Item II (b) of Resolution No. 2003-159 (A) dated December 1, 2003, which shall now read as follows:

b.1) Original and Photocopy of the Certificates of Live Birth printed on National Statistics Office (NSO) security paper and signed by Ms. Carmelita N. Ericta with attached original receipt.

b.2) Original and Photocopy of the computer generated NSO Certificates of Live Birth stating at the dorsal portion thereof, "A Certified True Copy: Office of the Civil Registrar General" duly signed by an authorized NSO statistician in security paper with attached original receipt.

b.3) In case a negative certification of no record of birth is issued by the NSO, the applicant shall have the Certificate of Live Birth issued by the local civil registrar in security paper authenticated by authorized NSO statistician.  The authenticated Certificate of Live Birth, attached to the NSO Certification on negative verification, shall be submitted by the applicant in applying for the licensure examination."

Let a copy of this resolution be published in the Official Gazette or newspaper of general circulation and furnished the U.P. Law Center, the Commission on Higher Education (CHED), all schools, colleges or universities offering courses for licensure examinations.

This Resolution shall take effect upon its approval.
