[ LTO MEMORANDUM CIRCULAR NO. AHS-2008-273, December 18, 2008 ]

UNIFORM MEDICAL CERTIFICATE



Pursuant to Administrative Order No. RIB-2007-012 dated 17 May 2007 revising the rules and regulations governing the supervision and control over accredited physicians, all medical certificates issued by any government or accredited physician shall be in the prescribed form using white bond paper as hereto attached.[*] The accredited physician should write legibly his findings/recommendations and shall use the terms that can be understood even by non-medical personnel. All entries including visual acuity, hearing, general physique, blood pressure, height, weight and presence or absence of contagious diseases shall be properly filled up. Comments/remarks as to other condition of the applicant affecting his fitness to drive should be indicated.

The Chief of LTO Licensing Centers and District Offices shall not accept medical certificates that do not conform to the prescribed form and not properly and completely filled up with the above described entries.

Further, the accredited physician is no longer required to check the kind of driver ™s license being applied for.

All orders and memoranda in conflict herewith are hereby deemed superseded.
