ESTABLISHING THE CRITERIA IN THE EVALUATION OF ACCREDITED PHYSICIANS PERFORMANCE



To ensure an effective and efficient public service and pursuant to Memorandum Circular No. RIB-2007-889 establishing the criteria in the evaluation of accredited physicians performance, in compliance with the ISO 9000-2001 requirements, the following monthly performance reports shall be submitted to the Central Office Committee on Accreditation of Physicians, to wit:

1. A monthly Performance Evaluation Report on each accredited Physician shall be submitted based on the ocular inspection conducted by the Regional Committee, in their respective area of jurisdiction to be accomplished in the prescribed form

2. A monthly Report on all Medical Examinations conducted by all accredited physician shall be submitted to the LTO Central Office Committee copy furnished the Regional Office Committee. The report shall also be made in the prescribed form.

Attached* herewith are the prescribed forms for Performance Evaluation of Accredited Physicians, Information Sheet, Letter of Application and Report on Medication Examinations conducted for information and dissemination.
