USE OF THE ORIGINAL AND LEGIBLE COPIES OF QUALITY CONTROL TEST REPORTS IN THE ACCEPTANCE AND PAYMENT OF THE MATERIALS
It has come to the attention of this Office that manufactured/fake duplicate or xeroxed copies of quality control test reports are being used to facilitate acceptance of materials and support claims for payments of contractors and suppliers. This practice will eventually result to the use of inferior or substandard materials in the project.
Henceforth, in order to stop this malpractice, only the original and legible copies of test reports should be used to form part of the supporting documents for every billing of the contractors and suppliers. As such, verified or authenticated copies of test reports should no longer be allowed to be used in the acceptance and payment of the materials.
Furthermore, if there is any doubt as to the authenticity of the original copy of the test reports, verification should be made with the Head of the concerned testing laboratory.
This supersedes Ministry Order No. 34, series of 1984, pertaining to the Authenticity of Quality Control Reports.
This order takes effect immediately
