ASSIGNMENT OF CLAIM NUMBERS



1. Docket Books will be opened by the Administrative Division and by each Field Team.

2. Applicants in the different regions will submit their applications Teams concerned in accordance with procedures prescribed in Memorandum Circular No. 90-1.  In cases where an individual applicant, Accredited Conta Officer or PVAO Contact Officer in any region finds it more convenient, he m submit the application by mail or by hand direct to the Administrative Divis PVAO, Manila.

3.      Each application will be entered in the docket book and assigned a c number consisting of thirteen characters (letters and numerals) arranged in grouping:  AA-A00-00-000000.

3.1.   The first two characters indicate the type of benefit being claimed:
