REVISED WARRANTIES OF ACCREDITATION FOR HEALTH CARE PROVIDERS



Based on the Department of Health's AO 147, s-2004 and AO 183, s-2004 on the revisions governing licensure of hospitals and accreditation of ambulatory surgical clinics as well as the implementation of the Senior Citizen Act of 2003, PhilHealth has revised the Warranties of Accreditation for both institutional and professional health care providers.

Health care providers applying for accreditation should submit the notarized revised warranties of accreditation together with their application form and other required supporting documents. Failure to submit the revised warranties of accreditation would result to non-accreditation in compliance with Section 63 (b) of the Implementing Rules and Regulations of the National Health Insurance Act (RA 7875) as amended which states that the Corporation shall determine the required documents to be submitted to comply with the requirements and conditions for accreditation.

Health care providers with valid accreditation must submit the notarized revised Warranties of Accreditation within ninety (90) days from the effectivity of this circular. Non-submission of said documents after the prescribed period, health care providers will be given notices by PhilHealth requiring them to submit a written justification for non-compliance. Subsequent violations may adversely affect the processing of the providers' claims.

This circular takes effect on November 1, 2005.
