1. Qualification Document Certificate (QDC)
2. Seafarers Identification & Record Book (SIRB)
3. Harbor Pilots License Certificates
4. Ratings obtained in the Harbor Pilot Examination
5. Major Patrons License Certificates
6. Minor Patrons License Certificates
7. Boat Captains Certificates
8. Motorboat Operators License Certificates
9. Marine Diesel Mechanics License Certificates
10. Accreditation Certificates issued to Training Centers

Authority to Accept Payment (ATAP) shall be secured from the Manpower Development Office.
