There are complaints received by this Office that there are district offices which do not accept forms downloaded at the LTO Website i.e. Application for Driver ™s License (ADL), Motor Vehicle Inspection Receipt (MVIR), etc. The reason why these forms were provided in the website is to help LTO clienteles acquire forms needed for their applications.
