[ LTO MEMORANDUM CIRCULAR NO. VPT-2010-1399, September 28, 2010 ]
ADDENDUM TO THE MEMORANDUM DATED 07 SEPTEMBER 2010 USE OF DRY SEAL ON ISSUED MEDICAL CERTIFICATES BY LTO ACCREDITED PHYSICIANS
One of the non conformities raised during the Surveillance Audit of 30 June 2010 for this Office s compliance to the ISO requirements was the absence of the LTO Accredited Physician s Clinic Dry Seal on the pro-format Medical Certificate as evidence that said document is from an authorized service provider.
In this connection, LTO Accredited Physician s Clinical Dry Seal, bearing the Physician s name, Accreditation Certificate Number and Place of Validity, must be stamped on the lower left portion of the Medical Certificates they issue to driver s license applicants.
However, Government issued medical certificates are not required the dry seal stamping but shall be required to bear the institution s Logo on the top portion of the revised medical certificate.
This Memorandum Circular shall take effect on 26 October 2010. For information and compliance.
