AN ORDINANCE REQUIRING ALL HOSPITALS AND BIRTHING HOMES IN QUEZON CITY TO SUBMIT TO THE QUEZON CITY HEALTH DEPARTMENT A MONTHLY REPORT ON THE NUMBER OF MATERNAL DEATHS, NEONATAL DEATHS, CHILD DELIVERIES AND LIVE BIRTHS AND OTHER RELATED INFORMATION WITHIN FIVE (5) WORKING DAYS OF THE SUCCEEDING MONTH AND PROVIDING FINE FOR VIOLATION THEREOF.

WHEREAS, the Quezon City Health Department is a frontline local institution primary responsible in the implementation of public health programs for the attainment of the Millennium Development Goal (MDG) in Quezon City;
WHEREAS, maternal deaths are highly preventable through effective public health programs such as family planning services, antenatal care and health facilities;
WHEREAS, accurate and update Information on maternal, Infant and neonatal mortality is an essential component to the success of vital public health programs;
WHEREAS, in order for the Quezon City Health Department to establish an accurate information on maternal, infant and neonatal mortality it is appropriate that hospitals and birthing homes be inquired to submit a monthly report on maternal, infant and neonatal mortality to the Quezon City Health Department,
BE IT ORDAINED BY THE CITY COUNCIL OF QUEZON CITY IN REGULAR SESSION ASSEMBLED;
SECTION 1. All hospitals and birthing homes in Quezon City are hereby required to submit a monthly report to the Quezon City Health Department on the number of maternal deaths, neonatal deaths, child deliveries, and live births and other related information.
SECTION 2. The monthly report shall be submitted to the Quezon City Health Department within five (5) working days of the succeeding month.
SECTION 3. Any hospital or birthing homos which fails to submit the report as required under this Ordinance shall be meted a fine as, follows:
SECTION 4. The Quezon City Health Department shall formulate Implementing Rules and Regulations for the proper implementation of this Ordinance, which shall contain, among others, the form and required data to be submitted in connection with this ordinance.
- For the first offense, the hospital administrator or birthing home owner shall be fined in the amount of One Thousand Pesos (P1,000.00) and required to submit the report;
- For the second offense, the hospital administrator or birthing home owner shall be fined in the amount of Two Thousand Pesos (P2,000.00) and required to submit the report;
- For third and succeeding offenses, the hospital administrator or birthing home owner shall be fined in the amount of Five Thousand Pesos (P5,000.00) and required to submit the report;
SECTION 5. This ordinance shall take effect upon its approval.
